Job summary

LaSante Health Center in Brooklyn is seeking an Internal Medicine Provider to join our multispecialty, community-based health center. You will deliver comprehensive primary and preventive care to adults, manage chronic conditions, and promote wellness in a culturally diverse community.

Qualifications

• MD/DO/NP/PA degree from an accredited institution.
• Board certification or eligibility in Internal Medicine or Adult Primary Care.
• Current New York State license or eligibility for licensure.
• Experience in community health or outpatient primary care preferred.
• Excellent communication and interpersonal skills.
• Bilingual Creole/English strongly preferred.

Responsibilities

• Provide primary and preventive care for adult patients, including physical exams, screenings, and chronic disease management.
• Diagnose and treat acute and chronic medical conditions.
• Collaborate with multidisciplinary teams to coordinate patient-centered care.
• Counsel patients on lifestyle modification, disease prevention, and medication management.
• Maintain accurate, timely documentation in the electronic health record (EHR).
• Participate in quality improvement initiatives and team meetings.
• Complete additional tasks as directed by supervisor.
