You will be the clinical quarterback, leading a multidisciplinary team across centers, telehealth, and home visits. Administrative days can be done remotely, with a competitive base salary and performance-based

Qualifications

• Provide longitudinal primary care to older adults with complex needs.
• Collaborate with care team to address medical and social determinants.
• Deliver care in patients' homes, centers, and via telehealth.

Responsibilities

• Clinic and telehealth visits with your panel.
• Care team huddles with Guides and Medical Technician Partners.
• Remote documentation and care coordination.
• Home-based care coordination with the MTP team.
