Job summary

HHM Health in Dallas, TX seeks an energetic Internal Medicine - Geriatric Physician to provide comprehensive primary and geriatric care for adults and older adults. You will work as part of an interdisciplinary team to coordinate care across settings.

The ideal candidate has an MD or DO, completed Internal Medicine residency, board certified/eligible, Texas license, and experience with value-based care and EHR systems (eClinicalWorks preferred).

We are seeking an energeticPhysician to join our growing team of providers. The Internal Medicine - Geriatric Physician provides comprehensive, patient-centered primary and geriatric care to adult and older adult patients within HHM Health. The physicianis responsible forthe prevention, diagnosis, treatment, and longitudinal management of acute and chronic medical conditions, with particular attention to the complex medical, functional, cognitive, and psychosocial needs of older adults. The physician works as part of an interdisciplinary care team to improve health outcomes, promote healthy aging, preserve independence, reduce avoidable hospital and emergency departmentutilization, and ensureappropriate coordinationacross the continuum of care.

  • Provide comprehensive primary care to adult and geriatric patients, including preventive, acute, and chronic care.
  • Diagnose and manage complex conditions such as hypertension, diabetes, cardiovascular, kidney, pulmonary, and musculoskeletal diseases.
  • Perform comprehensive geriatric assessments addressing medical, functional, cognitive, behavioral, and social needs.
  • Develop individualized, evidence-based treatment plans and provide age-appropriate screenings, immunizations, and health counseling.
  • Assess and manage geriatric syndromes including falls, frailty, dementia, delirium, incontinence, malnutrition, and functional decline.
  • Review medications for effectiveness, interactions, adverse effects, and polypharmacy concerns.
  • Support advance care planning, goals-of-care discussions, and appropriate palliativecare for patients with serious or advanced illness.
  • Order, interpret, and follow up on appropriate laboratory, imaging, and diagnostic studies and make necessary specialist referrals.
  • Provide timely follow-up after emergency visits, hospitalizations, and other transitions of care.
  • Collaborate with interdisciplinary teams, including APPs, nurses, pharmacists, behavioral health, social workers, and care coordinators.
  • Coordinate care with hospitals, specialists, home health, rehabilitation, skilled nursing facilities, and community partners.
  • Identify social determinants of health and connect patients with appropriate community and supportive resources.
  • Participate in population health initiatives focused on chronic disease management, preventive care, medication adherence, and improved outcomes.
  • Support initiatives to reduce avoidable emergency department visits, hospitalizations, and readmissions while promoting aging in place and functional independence.
  • Maintain accurate and timely EHR documentation while following organizational standards for coding, chart completion, and clinical workflows.
  • Support UDS, HEDIS, HRSA, quality improvement, peer review, and other clinical performance initiatives while maintaining regulatory compliance.
  • Provide compassionate, culturally responsive, patient-centered care and engage patients and families through shared decision-making.
  • Build long-term therapeutic relationships by communicating clearly, supporting caregivers, and promoting continuity of care while respecting patient autonomy and confidentiality.

What you need to succeed

  • MD or DO degree from an accredited institution and successful completion of an accredited Internal Medicine residency.
  • Board certified or board eligible in Internal Medicine with a current, unrestricted Texas medical license or eligibility to obtain one.
  • Current DEA registration or eligibility to obtain registration, along with current BLS certification.
  • Must be eligible for credentialing and privileging with HHM Health, hospitals, health plans, and applicable governmental programs.
  • Geriatric Medicine fellowship training or board certification/eligibility is preferred, along with experience caring for medically complex older adults.
  • FQHC, community health center, safety-net, or underserved healthcare experience is strongly preferred.
  • Experience with value-based care, population health, quality improvement, interdisciplinary care, and EHR systems; eClinicalWorks experience preferred.
  • Strong knowledge of Internal Medicine, geriatric care, chronic disease management, multimorbidity, and medication/polypharmacy management.
  • Excellent clinical judgment, communication, organization, documentation, and teamwork skills, with a commitment to health equity and patient safety.
  • Ability to perform outpatient clinical duties, communicate effectively with patients and care teams, use clinical technology, and travel between HHM Health locations or community settings as needed.
