HHM Health in Dallas, TX is seeking an Internal Medicine – Geriatric Physician to provide comprehensive primary and geriatric care for adults. You will manage prevention, diagnosis, treatment, and longitudinal care within a collaborative care team.

The role emphasizes aging in place, complex comorbidity management, and coordination with hospitals and community partners to improve outcomes.

Qualifications

• MD or DO degree from an accredited institution and successful completion of an Internal Medicine residency.
• Board certified or board eligible in Internal Medicine with a Texas license or eligibility.
• Current DEA registration and BLS certification; credentialing and privileging eligibility.
• FQHC/underserved healthcare experience preferred; value-based care and population health knowledge.

Responsibilities

• Provide comprehensive primary care to adult and geriatric patients, including preventive, acute, and chronic care.
• Diagnose and manage complex conditions such as hypertension, diabetes, cardiovascular and multisystem diseases.
• Perform comprehensive geriatric assessments addressing medical, functional, cognitive, behavioral, and social needs.
• Develop evidence-based treatment plans and provide age-appropriate screenings and immunizations.
• Coordinate with interdisciplinary teams and follow-up after transitions of care.
