Job summary

Endeavor Health seeks an Advanced Practice Provider (APN/PA) to join our Internal Medicine team at Elmhurst Health Center #3 in Elmhurst, IL. The role is outpatient, full-time, with a typical 40-hour workweek and on-call rotation as needed.

You will gather histories, perform examinations, order tests, interpret results, and manage acute and chronic conditions within established protocols. Collaboration with physicians and health care teams is essential to deliver high-quality patient care.

Qualifications

• IL Advanced Practice Licensure in IL required.
• Master’s level program in specialty area completed.
• BLS/CPR certification required.
• Prior APN/PA outpatient experience preferred.

Responsibilities

• Provide care to patients under the direction of collaborating physicians.
• Take detailed history, perform physical examinations, order imaging/lab tests and interpret results.
• Instruct patients on therapies and post-operative instructions.
• Maintain records and progress notes; follow patients through treatment in clinic and hospital settings.
• Prescribe therapies under protocol and participate in standards of practice development and evaluation.

• Hours: Hours are variable and flexible, based on department needs and candidate availability.
• Physicians and APPs have an on-call rotation during the week/weekend/Holiday call

• Exciting opportunity to join Endeavor Health in the Internal Medicine Department.
• Under the direction of the collaborating physician(s), provides care to patients.
• Obtains detailed history and performs physical examinations, orders imaging, lab tests and diagnostic procedures and interprets results.
• Instructs patients on the prescribed therapeutic regimen and pre/post operative instructions.
• Maintains records and writes progress notes on status, treatments and procedures.
• Follows patients through their course of treatment in the clinic and hospital setting.
• Collaborates with physicians and the health care team to achieve optimal patient outcomes.
• Prescribes pharmacologic and non-pharmacologic therapies as appropriate.
• Demonstrates evidence-based practice.
• Participates in the development, implementation, and evaluation of standards of practice in the specialty.
• Assesses the learning needs of patients and families across the continuum.
• Employs strategies, interventions and teaching to promote, maintain, and improve health.
• Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease.
• Promotes wellness and prevent illness and injury in various settings for individuals, families, and communities.
• Works autonomously and in interdisciplinary teams as resources and consultants.
• Positively contributes to safety outcomes and promoting high quality patient experience.
• Participates in the development, implementation, and evaluation of service line goals.
• Supports the strategic plan of the organization.
• Adheres to all Endeavor Health policies and procedures.
• APP will see new pts, established pts, walk ins, etc.) If APP has specific procedures that might be done the office will work to ensure supplies and ability to do so are completed.

Qualifications

• License: Advanced Practice Licensure (APN) or Physician Assistant (PA) Licensure in the State of IL required; Federal DEA and Illinois Controlled Substance License required
• Education: Completion of master’s level program in specialty area
• Certification: BLS/CPR certification required
• Experience: Prior APN/PA experience preferred
• Experience working in an outpatient or community health setting
