Endeavor Health in Elmhurst, IL is seeking an Advanced Practice Provider (APN/PA) for outpatient Family Medicine. The role involves seeing new and established patients, walk-ins, and coordinating with physicians in a collaborative team.

The position is full-time (40 hours/week) with rotating outpatient call and potential sign-on bonus for experienced candidates. The successful candidate will have IL APN/PA licensure (DEA and IL CS license), a Master’s degree in the specialty area preferred, and

Responsibilities

• Provide care to patients under physician supervision.
• Conduct histories, physical exams, order imaging/lab tests and interpret results.
• Prescribe therapies and develop care plans; promote wellness and preventive care.
• Work autonomously and in interdisciplinary teams to ensure high-quality patient care.
• See new patients, established patients and walk-ins in an outpatient setting.

$56.44 - $70.58 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

Job Description

Exciting opportunity to join Endeavor Health in the Family Medicine Department. Under the direction of the collaborating physician(s), provides care to patients. Obtains detailed history and performs physical examinations, orders imaging, lab tests and diagnostic procedures and interprets results. Instructs patients on the prescribed therapeutic regimen. Maintains records and writes progress notes on status, treatments and procedures. Collaborates with physicians and the health care team to achieve optimal patient outcomes.Prescribes pharmacologic and non-pharmacologic therapies as appropriate. Demonstrates evidence-based practice. Participates in the development, implementation, and evaluation of standards of practice in the specialty. Assesses the learning needs of patients and families across the continuum. Employs strategies, interventions and teaching to promote, maintain, and improve health. Provides comprehensive health assessments, formulates differential diagnoses, and prescribes treatments under the protocol in the direct management of acute and chronic illness and disease. Promotes wellness and preventative health in the outpatient setting. Works autonomously and in interdisciplinary teams as resources and consultants. Positively contributes to safety outcomes and promoting high-quality patient experience. Participates in the development, implementation, and evaluation of service line goals. Supports the strategic plan of the organization. Adheres to all Endeavor Health policies and procedures. This advanced practice provider will see new pts, established pts, walk ins, etc.

Qualifications:

• License: Advanced Practice Licensure (APN) or Physician Assistant (PA) Licensure in the State of IL required; Federal DEA and Illinois Controlled Substance License required
• Education: Completion of master’s level program in specialty area preferred
• Certification: BLS/CPR certification required
