On Deck Healthcare in Los Angeles, CA, seeks an experienced Interim Case Management Manager to lead care transitions and discharge planning for a hospital. This full-time onsite role supports staff while a permanent search is conducted.

You will partner with physicians and multidisciplinary teams to ensure safe, efficient patient flow, staff engagement, and adherence to regulations. Start within 2–4 weeks; travel home as needed.

This role is ideal for a steady, clinically strong leader who can step in to stabilize and support staff while the hospital conducts a permanent search.

Key Responsibilities:
