Foremost at Sharon in Sharon, MA is seeking a Director of Social Services / Social Services Coordinator to lead discharge planning and psychosocial care for residents in our skilled nursing facility. You will work with an interdisciplinary team to ensure smooth transitions and individualized support aligned with care plans.

The role requires a BSW and MA licensure (LSW/LCSW preferred), 1–3 years in geriatric social services, and strong Medicare/Medicaid knowledge.

Qualifications

• Bachelor’s Degree in Social Work or related field; MSW preferred.
• Licensed Social Worker (LSW) or Licensed Certified Social Worker (LCSW) in MA preferred.
• 1–3 years of social services experience in skilled nursing or geriatric care.
• Strong knowledge of Medicare, Medicaid, and MDS assessments.

Responsibilities

• Discharge planning coordination with residents, families, therapists, and staff.
• Conduct psychosocial assessments and develop individualized care plans.
• Advocate for residents and families, provide counseling and crisis support.

Job Title: Director of Social Services / Social Services Coordinator

Location: Foremost at Sharon (Sharon, MA)

Position Type: 24-32 flex hours

Position Summary

The Director of Social Services is responsible for identifying and meeting the psychosocial, emotional, and discharge-planning needs of our residents. Working as a key member of our interdisciplinary care team, this role ensures that residents smoothly transition into our skilled nursing facility, receive ongoing support tailored to their individual care plans, and are successfully discharged back home or to an alternative community setting.

Key Responsibilities

• Discharge Planning: Coordinate comprehensive discharge and transition plans in collaboration with residents, families, physical/occupational therapists, nursing staff, and external community agencies.

• Psychosocial Assessments & Care Plans: Conduct initial and ongoing psychosocial evaluations of residents. Develop, implement, and update individualized care plans that align with state and federal regulations.

• Resident & Family Advocacy: Act as a liaison between residents, families, and facility staff. Provide supportive counseling and crisis intervention to help residents and families adjust to long-term care or short-term rehabilitation.

• Resource Coordination: Connect residents and their families with financial, legal, and community resources, including Medicaid/Medicare navigation, local Aging Services Access Points (ASAPs), and support groups.

• Regulatory Compliance: Maintain thorough, accurate documentation in electronic medical records (EMR) in compliance with Massachusetts Department of Public Health (DPH) and federal CMS standards.

• Interdisciplinary Team Participation: Attend regular care plan meetings and daily morning huddles to report on resident progress, discharge barriers, and psychosocial updates.

Qualifications & Requirements

• Education: Bachelor’s Degree in Social Work (BSW), Psychology, Gerontology, or a related human services field (Master’s in Social Work / MSW preferred).

• Licensure: Licensed Social Worker (LSW) or Licensed Certified Social Worker (LCSW) in the Commonwealth of Massachusetts is preferred.

• Experience: Minimum of 1 to 3 years of social services experience within a skilled nursing facility, sub-acute rehabilitation center, or geriatric healthcare setting.

• Knowledge Base: Strong working knowledge of Medicare, Medicaid, MDS (Minimum Data Set) assessments, and local Massachusetts healthcare/aging networks.

• Skills: Exceptional empathy, patience, active listening, and strong written/verbal communication skills. Ability to manage multiple priorities in a fast-paced environment.
