The role leads a multidisciplinary home health team, manages staff assignments, coordinates patient care, ensures compliance with regulatory standards, and drives operational efficiency while delivering high-quality home health services.

Qualifications

• Bachelor's degree in Nursing (BSN) preferred.
• Current and active California RN license required.
• BLS Certification required.
• Minimum of 2 years of clinical leadership experience required.

Responsibilities

• Oversees a multidisciplinary home health team (RNs, LVNs, therapists, social workers, home health aides).
• Manages case assignments, distributes workflow resources, coordinates with physicians, and monitors productivity targets.
• Ensures all patient care delivery models adhere to the Nurse Practice Act, The Joint Commission, and federal/state home health regulations.
• Translates regional operational objectives into tactical daily action plans and resolves clinical escalation or HR issues.
• Investigates and mitigates patient, member, or family complaints regarding home medical services.
