Job summary

Appalachian Community Services in Western North Carolina seeks an experienced Intensive In-Home Team Lead to guide a team of clinicians, provide direct therapy, and ensure evidence-based care for youth and families.

The role requires a Master’s degree in a relevant field, full licensure, and a commitment to collaborative, strengths-based care. Travel within the service area is expected, with a competitive benefits package.

Qualifications

• Master’s degree in Social Work, Mental Health Counseling, Marriage & Family Counseling.
• Full licensure or Associate licensure as LCSW, LCSWA, LCMHC, LCMHCA, LMAFT, LMFT or equivalent.

Responsibilities

• Conduct and oversee client assessments to determine clinical needs and outcomes.
• Assign team members based on clinical expertise to ensure high-quality care.
• Provide individual and family therapy to Enhanced Services clients.
• Ensure service delivery aligns with PCPs and medical necessity criteria.
• Monitor and evaluate interventions and client progress to support positive outcomes.
• Provide clinical supervision and guidance to team members.
• Review service notes and PCPs for compliance.
• Lead regular team meetings to discuss client progress and challenges.
• Facilitate crisis response and linkage to additional services as needed.
• Travel within the service area and maintain a flexible schedule.
