Job summary

Responsibilities

• Provide intensive home‑based therapy and support to a caseload of 4–6 youth/families.
• Deliver services in homes, schools, and community settings.
• Provide crisis intervention and participate in an on‑call rotation.
• Assess safety concerns and develop individualized safety plans.

Skills

Experience working with youth, families, and/or groups with behavioral health needs

What You’ll Do

• Provide intensive home‑based therapy and support to a caseload of 4–6 youth/families


• Deliver services in homes, schools, and community settings


• Provide crisis intervention and participate in an on‑call rotation


• Assess safety concerns, risk behaviors, and develop individualized safety plans


• Utilize evidence‑based approaches including trauma‑informed care, motivational interviewing, and family systems treatment


• Support families in building communication, coping, and problem‑solving skills


• Coordinate care with internal teams, schools, and community providers


• Advocate for clients and connect them to needed resources


• Maintain timely and accurate documentation in compliance with agency standards


• Participate in supervision, team meetings, and ongoing training

What We’re Looking For
