Easter Seals Vermont, Inc. is seeking an Intensive Family Based Services Coordinator to deliver intensive in-home and community interventions for children, youth, and families at risk of state custody or in reunification.

The role requires a relevant degree or significant experience, CANS certification, and a valid driver’s license. Responsibilities include tracking referrals within a 13‑week timeframe, signing Plans of Care, coordinating with DCF-FSD, and maintaining CANS certification.

Qualifications

• Bachelor's Degree in human services or related field, OR four years' relevant experience with an Associate’s Degree.
• High School diploma with eight years of relevant experience.
• Two years of experience at Easterseals or in the human services field preferred.
• CANS certification or ability to obtain certification within three months.
• Valid driver’s license and reliable transportation.
• Previous supervisory or leadership experience strongly preferred.

Responsibilities

• Ensure IFBS Coordinators provide intensive, short term interventions in home and community to referred children, families and youth.
• Track referrals to adhere to the 13 week program timeframe and manage waitlists.
• Sign Plans of Care and assist with CANS assessments and review documentation regularly.
• Assign referrals and maintain consistent communication with DCF-FSD.
• Take referrals and maintain a minimal caseload to meet contract obligations and support staff.
• Maintain CANS certification.
• Support ongoing training and use of Motivational Interviewing in all referrals.
• Attend monthly IFBS provider meetings and trainings to adhere to contract requirements.
