Job summary

Easterseals Vermont seeks an Intensive Family Based Services Coordinator in Morrisville to support families at home and in the community. You will write and implement plans of care, prepare notes and discharge summaries, and stay in close contact with Family Service Workers to ensure adherence to each family’s plan.

The role emphasizes helping families access housing, transportation, counseling and nutrition services, while coordinating with schools and other providers to improve family

Qualifications

  • Bachelor’s degree in a related field is required.
  • Two (2) years of experience preferred.
  • Excellent writing skills are necessary.
  • Valid driver’s license and reliable transportation are required.

  • Provide in-home family support during times that work for the family.
  • Coordinate services with Schools, Early Intervention services, clinicians and other providers.

  • Help caregivers to recognize their children’s needs, and make plans and implement strategies so that they meet their children’s needs and grow in their confidence and skills, while providing a safe environment for their children.
  • Participate in meetings, conferences, trainings events as assigned.
  • Provide transportation for clients when required.
  • Testify concerning services in court when subpoenaed.
  • Ensure Family Assessments are done within a timely manner as prescribed by IFBS policies
  • Create crisis plans and respond to crisis calls as needed
  • Provide in home family support during the times that work for the family.
  • Implement short term (13 week) intervention and support in an effort to prevent DCF involvement or out of home care, and/or to reunify children with their family; to improve family functioning.
  • Coordinate services with other providers, such as Schools, Early Intervention services, clinicians and other relevant service providers.

We are looking for an Intensive Family Based Services Coordinator to join our team in Morrisville, Vermont! The individual in this key position will be responsible for the following:

Compensation

Starting $23.50 per hour. Additional compensation based upon years of related experience.

Hours

This is a full-time, 40 hour/week position.
