Job summary

HELP of Southern Nevada is seeking a New Beginnings Intensive Case Manager to provide direct, intensive case management to chronically unhoused adults, guiding them toward permanent housing and self-sufficiency.

You will conduct intakes, develop individualized plans, and connect clients to wrap-around services with HELP and partner agencies. Strong communication and data-entry skills are essential.

Qualifications

• Knowledge of local social service agencies and issues relating to unhoused populations.
• Experience with interviewing and referral processes.
• Strong written, verbal, and listening skills; ability to communicate across diverse populations.

Responsibilities

• Provide direct intensive case management to chronically unhoused adults under supervision.
• Conduct intake and needs assessments.
• Develop individualized case plans and review monthly.
• Coordinate wrap-around services and referrals to housing, energy assistance, and other programs.
• Enter case notes daily in HMIS and maintain audit-ready files.
• Perform income calculations and rent/utility cost analyses.
• Conduct weekly and semi-monthly home visits; provide transportation for appointments.
