Job summary

The Diocese of Camden seeks an Intensive Case Manager to support clients in Camden County as they approach the end of TANF/GA benefits. You will engage, assess, and intervene to help clients pursue employment or determine barriers, meeting with them at least twice a month for up to 24 months.

Based at Catholic Charities, you will collaborate with WFNJ case managers, ensure timely documentation, and communicate progress to the State Division of Family Development.

Qualifications

• Bachelor’s degree in social work or related field.
• Experience providing case management services.
• Knowledge of Trauma‑Informed Care and/or Motivational Interviewing is beneficial.

Responsibilities

• Reflect the mission and vision of Catholic Charities in every encounter with and on behalf of clients.
• Receive and review new client intake information from the County Social Services Agency monthly.
• Ensure clients are aware of scheduled intake and appointments via phone, email or mail.
• Conduct in-person comprehensive client social assessment for each client referred monthly.
• Assist clients in developing a strengths-based ISP toward employment or removing barriers.
