Job Summary

Under the supervision of theProgram Lead, the Intensive Care Facilitator is responsible for the coordination, scheduling and facilitation of Child and Family Team (CFT) meetings. The Intensive Care Facilitator will conduct regular CFT meetings for each client on their caseload within the timeframe identified by the program and/or needs of the client/family. They will complete associated documentation including the CFT Matrix and progress note. This position will collaborate with inter and intra-agency providers to coordinate care, monitor and ensure resources and linkages are accessible and appropriate for the youth and family. They are well versed in and are able to communicate to others the program philosophy, principles, and process. Services are provided primarily in the field including at clients’ residence, school, other community location, or in the office. The Intensive Care Facilitator is responsible for completing all required documentation in accordance with Agency, County, and COA standards.

Essential Functions

  • Coordinate CFT Meetings to develop, guide, and plan service delivery to meet the needs of the client and family
  • Utilizes the Integrated Core Practice Model (ICPM) principles and components to engage with participants, identify strengths and underlying needs, and strengthen teaming across systems
  • Coordinate comprehensive care including facilitating cross-system communication, planning and linking participants to resources and other child-serving systems
  • Collaboration with community partners, formal and informal supports including, but not limited to, DMH, DCFS, Department of Probation, Regional Center, DPSS, etc.
  • Ensure plans from all involved child services systems are integrated and identified goals and objectives are supported to support successful change
  • Ensures the CFT includes the client, family members, clinical team members, County representatives, and natural supports identified by the client and family
  • Works to ensure active participation in the CFT process by all members
  • Works with family and team to identify strategies for achieving treatment goals
  • Develops CFT meeting agenda reflective of the voice of the client and family, Matrix, progress note and other supporting documentation
  • Responsible for scheduling and managing the logistics of the CFT Meeting Process including location, time, materials, etc.
  • Routinely monitor, review progress and the effectiveness of treatment plans and services, working with the team to modify interventions that are not working
  • Identify and provide needed resources and linkages
  • Follow up on referrals, linkages, activities and outcomes to ensure client needs are met
  • With the team, develop transition plan that supports long-term stability
  • Complete and collect signatures of all attendees of CFT meetings, provide copies of Matrix
  • Ensure CFT meeting maintains fidelity to the ICPM
  • Build consensus within the team around collaborative plans
  • Ensure the family voice and choice is heard throughout the entire teaming process
  • Ensure all identified services are accessed, coordinated and delivered in a strength-based, individualized, family/youth driven, culturally and linguistically competent manner
  • Actively participate in scheduled staff meetings, agency wide meetings, and other meetings as assigned
  • Sensitive and responsive to the cultural differences among the agency’s client population
  • Actively participate in quality improvement projects
  • Adheres to all applicable Federal and State regulations for confidentiality, HIPAA, mandated reporting of suspected child or elder abuse

The Intensive Care Facilitatorwill take on special projects or additional duties as assigned by the Program Lead.

Qualifications

Competency Requirements

-Ability to speak, read, write and understand English

-Bilingual (English/Spanish) required

-Ability to work effectively as a member of the treatment team

-Ability to successfully work with families of diverse ethnic and cultural backgrounds

-Must have excellent oral and written communication skills

-Skilled in relationship building, conflict mediation and facilitation a plus

-Must have strong organizational and planning skills

-Knowledge of social services system and community resources

-Must have excellent interpersonal skills

-Demonstrate skill in operating Microsoft Word, Outlook and Excel

Education and Experience

-Bachelor’s degree in Social Work, Sociology, Psychology or related field

-Four years of relevant experience preferred

-Experience in crisis management preferred

-Experience with DMH services preferred

Certifications/Licenses

-Valid CA driver’s license

-Valid automobile insurance

Additional Requirements

-Tuberculosis clearance

-Department of Justice/Fingerprint clearance

-Satisfactory completion of health screening

-Proof of legal eligibility to work in the United States

-Satisfactory driving record

Physical Demands

While performing the duties of an Intensive Care Facilitator, the employee is frequently required to walk, drive, sit, and use their fingers to manipulate a keyboard or writing instrument. The employee is occasionally required to stand and reach. The employee is rarely required to run, bend, kneel or lift objects weighing more than 15 pounds.

Work Environment

The work environment is indoors and outdoors. The noise level varies from moderate to loud. The work environment is community-based and requires conducting home visits.
