Job summary

North London NHS Foundation Trust is seeking a Substance Misuse Recovery Partnership Worker within Enable Drug and Alcohol Services. The post strengthens integration between substance misuse services, social care and partner agencies to promote holistic care for individuals and families affected by substance use.

You will deliver patient-centred interventions including screening, assessments, care planning, brief interventions and harm reduction, while supporting safeguarding, risk management

Qualifications

• Deliver screening, assessment, and care planning for individuals with substance use needs.
• Provide brief interventions, harm reduction advice, and supported referrals to treatment and partner services.
• Manage a caseload of service users with complex needs, including those in opioid substitute treatment, ensuring ongoing engagement and review of care plans.
• Work collaboratively with social care, healthcare, and community partners to support integrated care pathways and improve access to services.

Responsibilities

• Screening, triage and comprehensive assessments of service users including health and social needs.
• Provide brief interventions and harm reduction advice.
• Support safeguarding and risk management within multi‑agency settings.
• Develop and review care plans, ensuring ongoing engagement and follow-up.
