Position Overview:

Sentara Healthcare is hiring an Integrated Nurse Case Manager for the Eastern Middle Peninsula region of VA. This full-time, day-shift role offers excellent benefits and requires travel for face-to-face home visits in areas including Millers Tavern, Dunnsville, and Tappahannock. Applicants must reside in or near these locations.

The Integrated Care Manager (ICM) is a Registered Nurse responsible for providing comprehensive case management services to high-risk members, focusing on complex care needs to improve health outcomes through proactive coordination, care planning, and resource management.

Target Member Population Includes:

• High ER utilizers
• Recent hospital discharges
• Members with heart failure, COPD, or diabetes
• Individuals with Developmental Disability (DD) waivers

Key Responsibilities:

• Conduct clinical assessments via phone or in person to evaluate member needs
• Develop and monitor personalized care plans
• Identify at-risk members and coordinate timely interventions
• Support management of chronic illnesses and disabilities
• Participate in quality improvement initiatives and manage care gaps
• Handle authorizations and referrals within benefit guidelines
• Collaborate with medical and interdisciplinary teams
• Present cases in team meetings to ensure coordinated care
• Ensure compliance with standards and policies
• Resolve provider, claims, or service issues affecting care

Qualifications:

• Associates or Bachelors Degree in Nursing
• RN License (Compact/Multi-State required)
• Minimum 3 years nursing experience with a focus on case management
• Experience with high-risk populations and chronic conditions
• Strong communication, critical thinking, and organizational skills
• Flexibility for travel in the field
