Job summary

The Licensed Integrated Mental Health Therapist-BHN provides assessments, treatment/care planning and psychosocial therapy within a primary care clinic setting for a diverse patient population. Working as part of a Collaborative Care team with a Psychiatric Consultant and Primary Care Provider, you will promote symptom reduction and optimal outcomes.

The position requires a Master's degree in a relevant field and appropriate licensure (LMFT/LMHC/LicSW).

Qualifications

• Master's degree in Social Work, Counseling, Marriage & Family Therapy, or related field.
• Licensed LMFT, LMHC, LICSW or equivalent per HB 1724.

Responsibilities

• Assesses patients' behavioral health needs and develops individualized treatment plans.
• Screens for common behavioral health disorders using validated tools.
• Monitors symptoms and treatment side effects to promote symptom reduction.
• Supports psychotropic medication management in collaboration with PCPs.
• Tracks follow-up and clinical outcomes to re-engage patients.
• Makes care plan changes and appropriate referrals when needed.
• Manages behavioral health crises within a primary care setting.
• Provides flexible services via scheduled and same-day interventions.

Education
