Job Title: Integrated Case Manager

Location: Detroit, MI

Assignment Type: 13-week contract (with the option to extend)

Schedule: Monday – Friday

Position Overview:

We are currently seeking a highly Experienced Acute Care Inpatient Case Manager with strong discharge planning expertise to join our team. This role requires professionals who can step in with a minimal training, adapt quickly, and manage patients efficiently in a fast-paced Acute Care Environment.

Key Responsibilities:

• Perform comprehensive case management for acute inpatient populations.
• Lead and coordinate discharge planning to ensure smooth transition of care.
• Collaborate with physician, nursing, and interdisciplinary teams to optimize patient outcomes.
• Utilize EPIC EMR for documentation and care coordination.
• Deliver efficient patient-centered care while managing complex cases.

Requirements:

• Minimum 5 years of acute inpatient case management experience
• Recent (within the last 2 years) acre care discharge planning experience
• Strong computer skills with proficiency in EPIC EMR
• BLS Certification through AHA
• Nursing Degree (RN) with valid RN License OR a Master of Social Work (LMSW) with valid Michigan licensure
• Ability to quickly adapt and work independently in a high-volume acute care setting
