Job Title: Integrated Case Manager

Location: Detroit, MI 48202

Duration: 13 Weeks with a possible extension

Local Pay Range: $50 - $55/Hour on W2

Travel Pay Range: $2,100 - 2,330

Duties:

  • The Integrated Case Manager for Population Health is an interdependent member of the patient-centered care or treatment team.
  • Responsible for collaborative assessment, planning, facilitation, care coordination, evaluation, and advocacy to meet the comprehensive health needs of individuals and families.
  • Promotes patient safety, quality of care, and cost-effective outcomes through communication and available resources.
  • Addresses the needs of patients experiencing critical events or diagnoses requiring complex management and extensive resource utilization.
  • Provides services across ambulatory, inpatient, and health plan settings.
  • Conducts comprehensive assessments of patients and family/caregivers, including biomedical, psychological, social, and functional needs to evaluate recovery impact.
  • Develops personalized, patient-centered care plans to optimize care experience.
  • Engages patients and families through advocacy, communication, education, and resource facilitation.
  • Applies professional judgment, critical thinking, motivational interviewing, and self-management techniques to overcome barriers.
  • Provides counseling and interventions for treatment decisions and end-of-life planning, including Advanced Care Planning.
  • Coordinates safe and seamless transitions between care settings.
  • Advocates for appropriate service delivery within the patient’s health plan benefits.
  • Collaborates with care teams to manage complex medical and social needs and facilitates care conferences.
  • Continuously evaluates patient response and adjusts care plans as needed.
  • Participates in process improvement initiatives, including data collection, analysis, and follow-up interventions.
  • Facilitates interventions in cases involving abuse (child, domestic, elder, institutional) and sexual assault.
  • Supports departmental and organizational goals.
  • Performs other duties as assigned.

Skills:

  • Strong customer service and interpersonal skills with the ability to interact across all organizational levels.
  • Strong problem-solving, analytical, and decision-making abilities.
  • Proficiency in computer skills and systems.
  • Experience in discharge planning, home health care, rehabilitative medicine, community health, or managed care (preferred).
  • Knowledge of preventive service guidelines, clinical practice standards, behavior change theory, Medicare and Medicaid regulations, and case management principles.
  • Understanding of medical ethics and legal considerations in case management.
  • Awareness of social determinants of health and their impact on patient wellbeing.
  • Ability to facilitate access to community resources for diverse populations.
  • Strong organizational, planning, and multitasking skills to manage complex patient needs.
  • Compassionate approach with strong patient and family advocacy skills.

Education:

  • Nursing degree OR Master of Social Work (MSW).
  • Registered Nurse (RN) with a valid, unrestricted State of Michigan license OR
  • Licensed Master Social Worker (LMSW) with a valid, unrestricted State of Michigan license.
