Job summary

Many Rivers Whole Health in Great Falls, MT, seeks an Integrated Care Supervisor to lead a Care Coordination team and oversee integrated care services across physical, behavioral health, and social needs. This role provides clinical supervision, supports staff performance, and drives quality improvements aligned with CCBHC standards.

The ideal candidate is a registered nurse with supervisory experience, strong communication, and expertise in care coordination, EHRs, and person-centered care

Qualifications

• Current, valid Montana RN license in good standing.
• Minimum of two years of supervisory experience.
• Strong clinical assessment, critical thinking, and care coordination skills.
• Excellent communication, leadership, and problem-solving abilities.

Responsibilities

• Supervising and supporting Care Coordinators, Targeted Case Managers, and Care Managers.
• Providing clinical consultation and guidance for complex medical and behavioral health situations.
• Monitoring care coordination activities, documentation, and service delivery.
• Supporting transitions of care between hospitals, primary care, behavioral health, crisis services, and community resources.
• Partnering with healthcare providers to ensure coordinated, whole-person care.
• Identifying barriers to care and facilitating appropriate interventions and referrals.
• Monitoring quality metrics, outcomes, and performance indicators.
• Assisting with staff recruitment, onboarding, training, and professional development.
• Supporting compliance with federal, state, nursing, and CCBHC requirements.
• Developing and maintaining strong relationships with healthcare and community partners.
• Assisting leadership with program development, workflow improvements, and operational planning.
