Job summary

CHR is seeking a BHH Specialist for the Behavioral Health Home in Manchester, CT. This full‑time position (40 hours/week) focuses on care coordination, health promotion, and support for adults with complex medical and mental health needs through integrated care.

You will collaborate with the BHH team, assist with service access, transportation, and crisis support, and participate in ongoing professional development. A sign-on bonus is offered to eligible applicants.

Qualifications

• BA in human services is required.
• Two years experience in a mental health setting; experience working with primary care providers preferred.
• Valid driver’s license and CPR/First Aid Certification within 6 months of hire.

Responsibilities

• Care Coordination for BHH enrollees
• Health Promotion and wellness support
• Referral to community support services and resource navigation
• Collaboration with BHH primary nurse care manager and team to improve outcomes
• Assist in accessing medical and wellness services, transportation, and other supports
• Maintain documentation in compliance with agency standards and participate in staff meetings and trainings
