Job summary

Whole Person Care Clinic (WPCC) seeks an Enhanced Care Manager to oversee ECM services, coordinating care across medical, behavioral health, and social supports to achieve optimal health outcomes for clients.

You will develop and oversee comprehensive care plans with clients and families, advocate with providers and community resources, monitor progress, and apply trauma-informed, confidential care within a collaborative team.
