Job summary

Rochester Regional Health is seeking a Care Navigator at Genesee Mental Health Center in Rochester, NY to coordinate and support mental health care within the Integrated Care team, including PCP, BHC, and psychiatric providers.

The role involves screening, education, and coordination of referrals, with focus on treatment adherence and patient engagement. Reports to the Manager, Behavioral Health/Primary Care Integration and supports team decisions.

Qualifications

• Minimum of 2 years of experience in a human services related field, medical related field, customer service, or client/patient interfacing experience required.
• Associate’s degree required, bachelor’s degree preferred.
• Experience in coordinating care and working with integrated care teams a plus.

Responsibilities

• Coordinate and support behavioral health care with the patient, PCP, BHC, and other integrated care team members.
• Screen and assess patients for common mental health and substance use disorders.
• Provide patient education about mental health conditions and treatment options.
• Monitor patients for changes in symptoms, side effects, or complications.
• Support psychotropic medication management and adherence through the care team.
• Facilitate referrals to mental health or substance use treatment programs as clinically indicated.
• Participate in Integrated Care team meetings and communicate treatment recommendations.
• Engage patients and maintain the Integrated Care registry to track outcomes.
