Job summary

Rochester Regional Health System seeks a Care Navigator to join the Integrated Care team at St. Mary’s Campus, coordinating mental health care across PCPs, BHCs, and psychiatric providers. The role includes patient education, referrals, and supporting treatment plans within and beyond the clinic.

The position reports to the Manager, Behavioral Health/Primary Care Integration. Over-nights (7:00PM-7:00AM) on the St. Mary’s Campus, with a focus on engagement and outcomes in a collaborative setting.

Qualifications

• 2+ years experience in Human Services or related field or associate degree required, bachelor’s preferred.
• Experience in coordinating mental health care and patient education is expected.
• Ability to communicate effectively with an Integrated Care team.

Responsibilities

• Coordinate behavioral health care with the Integrated Care team.
• Screen and assess patients for mental health disorders and substance use concerns.
• Provide patient education on disorders and treatment options.
• Monitor patients for changes in symptoms and treatment effects.
• Assist with psychotropic medication management and adherence.
• Provide brief evidence-based interventions in primary care settings.
• Facilitate referrals to mental health programs as clinically indicated.
• Participate in Integrated Care team meetings and share treatment plans.
• Engage patients and ensure follow-up in care.
• Maintain patient registry with engagement and outcome data.
