Job summary

Sentara Health in Virginia Beach seeks an Integrated Care Manager Transitions of Care to coordinate care across the continuum and improve outcomes. This role requires residency in Virginia Beach or the Hampton Roads area and involves working in acute care, service lines, ambulatory/community, home health, or long-term care settings.

You will develop and implement comprehensive care plans based on assessments, promote appropriate utilization of health services, and collaborate with the care team

Qualifications

• 3 years of nursing experience (required).
• 3 years Case Management experience (preferred).
• PACE incumbents require a minimum of one year of frail/elderly population experience.

Responsibilities

• Coordinate comprehensive care and quality outcomes for patients across the continuum.
• Promote effective utilization and monitoring of health services; collaborate with healthcare team and patient/caregiver.
• Develop and implement a comprehensive care plan based on assessment and evaluation of patient/caregiver needs.
