Job summary

CareSource is seeking a Community Based Care Coordinator, Duals Integrated Care to manage coordination for dual-eligible beneficiaries through community resources and health services. The role emphasizes integrating medical and social supports to improve outcomes and quality of life for individuals with complex needs.

You will collaborate with interdisciplinary teams, educate members and families on Medicare/Medicaid benefits, and coordinate with providers to prevent readmissions.

Qualifications

• Nursing degree from an accredited program or Bachelor's in a health care field.
• RN or Social Worker licensure required; relevant experience may substitute for education.
• Case management and care coordination experience with dual-eligible populations preferred.

Responsibilities

• Engage with members in community settings to establish care coordination relationships.
• Coordinate between healthcare providers and community resources to ensure smooth transitions.
• Develop individualized care plans addressing medical, social, and behavioral health needs.
• Monitor health status and adherence to care plans; adjust as needed.
• Travel to member homes, clinics, and community sites as required by the role.
