Job summary

Bitterroot Health is seeking an Integrated Behavioral Health (IBH) Care Manager to collaborate with PCPs in a primary care clinic in Hamilton, Montana. The IBH Care Manager will coordinate and advocate for patients' mental and physical health, functioning as a crucial member of the collaborative care team with a psychiatric consultant.

Based out of the Corvallis Clinics, this full-time position may involve responsibilities in Stevensville as well, and offers the opportunity to impact community

Qualifications

• Bachelor's Degree required, preferably in psychology, social work, public health, or some other field related to behavioral health.
• Licensure or licensure candidate preferred, but not required (e.g. LCSW, LCPC, LAC)
• 1 year of professional experience in a related field, required
• Experience with screening for common mental health and/or substance abuse disorders, preferred
• Working knowledge of differential diagnosis of common mental health and/or substance abuse disorders, preferred
• Experience with assessment and treatment planning for common mental health and/or substance use disorders, preferred
• Working knowledge of evidence-based psychosocial treatments and brief behavioral interventions for common mental health disorders, preferred
• Basic knowledge of psychopharmacology for common mental health disorders, preferred
• Experience with evidence-based counseling techniques, preferred

Responsibilities

• Collaborate with PCPs in a primary care clinic to support, promote, manage, coordinate, and advocate for the mental and physical health of patients on caseload.
• Be a crucial member of the collaborative care team, including the PCP, psychiatric consultant, and larger primary care team.
• Work from the Corvallis Clinics with responsibilities in Stevensville.

Bitterroot Health is seeking candidates to fill an integrated behavioral health (IBH) care manager position.IBH care managers are professionals (licensed or unlicensed) who work closely with primary care providers (PCP) in a primary care clinic setting to support, promote, manage, coordinate, and advocate for the mental and physical health of those patients assigned to their caseload.

An IBH care manager is a crucial member of the collaborative care team, which includes the patient’s PCP, a psychiatric consultant, and the patient’s larger primary care team.

This position will be based out of our Corvallis Clinics with responsibilities in Stevensville as well.

Minimum Qualifications:

Education:
