Job summary

Atlantic Health System is seeking a Behavioral Health Clinician to provide short-term therapy in our AMG Primary Care - Behavioral Health program in New Jersey. You will deliver individual therapy within the collaborative care model and coordinate with medical teams in Sussex and Warren counties.

The role emphasizes building rapport, conducting assessments, education on diagnoses and self-management, and applying evidence-based interventions.

Qualifications

• Master's Degree in Social Work, Counseling or related field.
• LCSW or LPC license.
• Previous clinical or counseling experience.

Responsibilities

• Establish rapport with patients through active listening, empathy, and non-judgmental communication.
• Screen, assess, and monitor patients for mental health and substance use disorders with follow-up.
• Educate patients and families on diagnoses, treatment options, relapse prevention and self-management.
• Provide brief, evidence-based behavioral interventions such as cognitive behavioral therapy and motivational interviewing.
• Collaborate with medical providers and psychiatric consultants on a shared plan of care.
• Facilitate referrals to evidence-based psychosocial treatments and social services.
• Develop, implement, and revise treatment goals with medical teams and validated screening measures.
• Track treatment response and safety issues via scheduled visits and caseload data.
• Identify and adjust treatment plans for patients needing more intensive services.
• Collaborate on relapse prevention and self-management plans near discharge.
• Maintain accurate, timely documentation in the EMR and ensure quality records.
• Ensure confidentiality and regulatory compliance.
• Meet productivity standards while contributing to program quality improvement.
