Qualifications

• Master’s degree in social work from an accredited university/college.
• Licensure in Indiana: LCSW, LMFT, or LCMHC.
• Basic Life Support (BLS) certification.

Responsibilities

• Provide comprehensive assessment and diagnosis of behavioral health patients.
• Assist providers in recognizing and treating mental, psychosocial, and substance use disorders.
• Provide treatment planning and assist patients in achieving goals.
• Evaluate crises and apply appropriate interventions and referrals.
• Maintain active communication with Primary Care Providers during clinic hours.
• Participate in meetings supporting the integrated care model.
