Iowa Primary Care Association, Inc. in Burlington, IA is recruiting a Behavioral Health Therapist to join a vibrant outpatient community health center.

The role operates within an integrated care model with dental, medical, and behavioral health services, using Epic EHR. The position supports a multidisciplinary team, offers full-time hours with 36 patient contact hours and 4 administration hours, and typically conducts 60-minute appointments.

Qualifications

• Graduation from an accredited master’s social work, counseling, marriage/family therapy and/or behavioral health education program.
• Licensed as behavioral health professional to practice as LISW in Iowa (LISW or LMHC)
• BLS, ACLS or ATLS certification required.

Responsabilités

• Provides psychosocial assessment to patients and families to identify emotional, social and environmental strengths and problems.
• Develops and implements a comprehensive psychosocial treatment plan with individual, marital, family or group therapies.
• Provides crisis intervention and emergency behavioral health services as needed.
• Participates in staff conferences to present case histories and coordinate with therapy team.
• Maintains case records describing problems and services provided.
• Educates patients and families on adaptation to diagnosis and treatment.
• Maintains liaison with community agencies and resources.
