Responsibilities

  • Work within a transdisciplinary primary care team to provide integrated behavioral health care and care management for patients with chronic conditions.
  • Coordinate mental health care with the patient’s medical provider and screen/assess for common mental health disorders.
  • Deliver brief, targeted treatments such as CBT and solution-focused therapy.
  • Advance care management tasks including pre-visit planning and patient self-management training.
  • Provide care coordination to address medical and behavioral health goals.
  • Communicate with physicians and care team about patient needs or changes.
  • Identify cost-effective protocols and contribute to care guidelines to improve population health.
  • Promote health equity by addressing disparities and outcomes.
