Job summary

Allegheny Health Network seeks a licensed behavioral health clinician to join the care team in managing the health of enrolled patients. The BHC provides counseling and co-management for mental health and substance use disorders within the scope of licensure, coordinating care and facilitating access to higher levels of care as needed.

Qualifications

• Master’s degree in Mental Health Field.
• 2 years experience in evidence-based counseling techniques, screening, diagnosis, and treatment of common mental health and substance abuse disorders.
• Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) or Licensed Marriage and Family Therapist (LMFT); for out of state license transfers allow 60 days from hire to transfer license

Responsibilities

• Provide mental health assessment and treatment planning within the care team.
• Deliver counseling interventions appropriate to ambulatory care using short-term approaches.
• Coordinate with patient Family members and support systems to facilitate care.
• Triage and refer patients to specialty mental health and substance abuse services when appropriate.
• Engage patients via telephone and in person to support treatment plans.
