Job summary

Bozeman Health seeks a Behavioral Health Specialist to develop and maintain a comprehensive program of behavioral health services for patients and families across the clinic setting. This role integrates with the multidisciplinary team to improve screening and management of social, emotional, and behavioral concerns and related medical conditions in ambulatory care.

Qualifications

• Master’s degree in Social Work, Psychology, Counseling, or related field.
• Licensed as an LCSW, LCPC, or LMFT in the state of Montana.
• Current American Heart Association (AHA) Basic Life Support (BLS) certification.
• Preferred: 3-5 years of experience in the health care field.
• Preferred: Relevant certifications

Responsibilities

• Support the mental and physical health care of patients across the care continuum. Closely coordinate care with the patient’s care team.
• Provide screening, brief intervention, and referrals.
• Document and maintain appropriate clinical documentation.
• Systematically track treatment response and monitor patients for changes in clinical symptoms and treatment side effects or complications.
• Support psychotropic medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment.
• Track patient follow up and clinical outcomes. Support maintenance of data registries.
• Provide or facilitate referrals to evidence-based psychosocial treatments (e.g. problem-solving treatment or behavioral activation) as clinically indicated.
• Facilitate referrals for clinically indicated services outside of the organization (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment).
• Other related duties and responsibilities as assigned.
