Job summary

The Collaborative Care Behavioral Health Care Manager (BHCM) at OSU-Center for Health Sciences is an integral member of the interdisciplinary primary care team in the Family Medicine Clinics.

The BHCM provides population-based behavioral health care within a Collaborative Care Model, collaborating with physicians, APPs, psychiatrists, psychologists, and care team members to support patients with depression, anxiety, and related needs.

Qualifications

• Master's degree in a behavioral health discipline is required.
• Experience delivering behavioral health services in outpatient/primary care settings is expected.
• Licensure or certification for the applicable discipline in Oklahoma is required.

Responsibilities

• Provide population-based behavioral health care as part of a Collaborative Care Model.
• Collaborate with primary care physicians, APPs, psychiatrists, psychologists and care teams.
• Engage in proactive outreach, measurement-based care, and follow-up across patients.
