Job summary

WellSpace Health is seeking an Integrated Behavioral Health Care Coordinator Medical Assistant at the J Street Center in Sacramento, CA. This full-time role supports primary care patients with behavioral health interventions, psychoeducation, case management, and coordination with Behavioral Health providers to improve outcomes.

The position requires a Medical Assistant credential or equivalent, CPR/CPR-AED, and bilingual abilities in Spanish preferred.

Qualifications

• High School Diploma or equivalent.
• Medical Assistant training/certification preferred OR an equivalent combination of education and experience.
• Current CPR or CPR/AED certification required.
• Bilingual in Spanish preferred; or other languages desirable.
• Commitment to HIPAA and ability to abide by standards of professional ethics and maintain confidentiality.
• Adherence to infection control procedures and HIPAA compliance.

Responsibilities

• Conducts warm hand-offs between the patient and referring primary care provider during health center hours to link patients with Behavioral Health Services.
• Assists patients in navigating the mental health system and appropriate linkage to various county agencies and other community resources based on patient needs and availability.
• Monitors and implements Patient-Centered Health Home (PCHH) goals.
• Monitors and implements Certified Community Behavioral Health Center (CCBHC) goals.
• Performs medical assistant duties, prepares rooms, rooms patients, records medical histories, vital signs, immunizations, and medication assistance.
• Documents all patient interactions in the Electronic Health Record according to WellSpace Health policies.
• Travels when necessary to support operational needs, meetings, and any function requiring presence.
• Maintains immunization files for the county registry.
