Job summary

Intermountain Centers in Tucson, AZ seeks a Care Coordinator to manage members' whole health using an integrated case management model, including assessment, planning, implementation, and monitoring.

The Care Coordinator will document in the Electronic Health Record, collaborate with primary care and interdisciplinary teams, advocate for resources, and ensure compliance with licensing and HIPAA policies while supporting member-centered care.

Qualifications

• Bachelor’s degree in behavioral health, social work, or related field is required.
• Associate’s degree plus 2 years related experience. OR high school diploma with 4 years behavioral health experience.
• Ability to read and interpret clinical information and coordinate care.

Responsibilities

• Use an integrated case management model to assess, plan, implement, and monitor services.
• Maintain member health records and treatment plans.
• Coordinate with primary care and interdisciplinary teams to improve member care.
• Document care and coordination in the Electronic Health Record (EHR).
• Provide services and coordination based on member needs and requests.
• Ensure HIPAA compliance and maintain professional conduct.
