INTAKE SPECIALIST - Jackson County

CareStar, Inc. is seeking an Intake Specialist in the Clinical Department to conduct program assessments using established tools and processes. The Intake Specialist will provide eligibility recommendations per program and CareStar guidelines.

The Position

CareStar is looking for motivated individuals who:

• Complete visits to assess eligibility for specific programs and conduct safety assessments in compliance with CareStar and department guidelines.
• Document all applicant and case-related contacts and interventions in the applicant's record per agency and CareStar policy, including standards of content and timeliness.
• Process Program Eligibility Assessment Tool (PEAT) or other tools and all related documents per agency and CareStar policy.
• Assist with community resource referrals as appropriate.
• Identify and report high-risk situations as required per agency and CareStar policy.
• Hold an active RN license.
• Have 12 months of experience in home and community-based services within the past 5 years.
• Possess knowledge of Medicare, Medicaid, Home Health Care, and Ohio Home Care waiver programs (desirable).
• Adhere to the CareStar Rule in performance of job responsibilities.
• Understand and comply with CareStar Policies and Procedures.
• Maintain confidentiality related to patient information; unlawful disclosures will be treated as serious disciplinary offenses.
• Follow the Acceptable Use Policy when using CareStar's information systems.

This position is based in Jackson County and covers surrounding counties.
