Job summary

Impact NW in Portland is seeking an Intake Specialist II (bilingual) to provide clinic-based community resource services at health centers. You will conduct brief client assessments, plan actions, and make individualized referrals to connect clients with needed services.

The role requires at least two years in social work or related fields, ability to relate to diverse communities, and a valid driver’s license for local travel. This is a full-time, on-site position in the Portland area.

Qualifications

  • Minimum of two years of work, volunteering in social work or related field, or lived experience.
  • One year of experience in working with families and individuals experiencing poverty and/or homelessness, or having long term mental health, addiction/recovery, and/or other long-term disabilities.
  • Sensitivity and ability to relate to people from diverse lifestyles, socioeconomic levels, and cultures with the abilities to provide culturally responsive services.

Responsibilities

  • Utilize person-centered and trauma-informed assessment to identify and prioritize participant needs.
  • Support and encourage participants self-advocacy.
  • Maintain compliance with all HIPAA standards.
  • Coordinate and/or assist with transportation planning as needed.
  • Collaborate with community partners to connect participants with food resources, energy assistance, housing assistance, medical/medication assistance, life skills classes, recovery resources and other resources as needed.
  • Maintain an up-to-date working knowledge of medical insurance benefits and assist participants with insurance application processes.
  • Bridge community, cultural, linguistic, education and other barriers to encourage participant engagement with community services.
  • Develop and maintain working relationships with health providers, nurse case managers and others to maintain effective outreach strategies and support participant access to the Resource Access Center Program.
  • Complete accurate, legible and timely documentation of services delivered in accordance with client information standards and systems, including the CRD database.
  • Meet client service standards set forth by Program Director and/or Program Supervisor regarding quantity, type and quality of services to be delivered to clients.
  • Comply with expectations and guidelines set forth by health clinic.
  • Provide leadership and facilitation in a variety of meeting settings.
  • Participate in agency and mandatory trainings.
  • Perform other duties as assigned.
  • Possess a valid driver’s license, a good driving record, and provide own dependable transportation to drive on behalf of agency.

JOB SUMMARY

We are currently looking for 1 Intake Specialists II (full-time), who is bilingual (ANY LANGUAGE) to provide clinic-based community resource services at health centers in one of the Portland locations.

Specific responsibilities include conducting brief client assessments, action planning, and individualized service referrals.

Specific duties include:

  • work with clinic staff to conduct program outreach
  • engage with participants to build rapport
  • evaluate participant needs
  • create individual action plans based on participants goals and objectives
  • provide referrals including a warm handoff to community agencies

  • Demonstrated ability to effectively work within a strength-based, trauma-informed, and person-centered support service delivery model.
  • Ability to relate to communities of color as well as with Impact NW’s diverse community of staff and volunteers.
  • Confidence and resourcefulness in crisis situations, and sensitivity to clients in vulnerable circumstances.
  • Ability to both - work independently and be self-motivated as well as willingness to work as a team member and support Impact NW’s mission.
  • Ability to communicate effectively in target foreign language.
  • Possession of consistent professional boundaries, good organizational skills and ability to prioritize multiple and varied tasks.
  • Ability to communicate effectively, both orally and in writing.
  • Working knowledge of Microsoft Office.
  • Possession of valid Driver License and up-to-date Vehicle Insurance Coverage.
  • Ability to efficiently travel/drive within the agency's service areas.
JOB CONDITIONS

Schedule Monday to Friday 8:30AM to 5:00PM. Repetitive motion, prolonged sitting, extensive visual involvement. May require some early morning, evening and/or weekend meetings. May be exposed to angry or distraught people.
