Intake RN — Mobile Care Coordinator for New Members

Job summary

CareSource in New York is seeking an Intake Nurse - RN to assess potential new members for eligibility and explain the program to them and their families. You will work with the Intake/Outreach and clinical teams to ensure appropriate services and a seamless transition to managed long-term care.

Responsibilities include community outreach, initial assessments with UAS, eligibility evaluations, and coordinating enrollment documentation; travel is required across the territory.

Qualifications

• Associates degree in Nursing is required; a Bachelor's degree is preferred.
• Minimum of 1 year of acute care experience is required.
• Home care, management, and long-term care experience preferred.
• Current NY Registered Nurse license is required; valid driver’s license may be required.

Responsibilities

• Conducts outreach in the community as needed.
• Performs the initial assessment using the UAS tool.
• Evaluates potential members for eligibility and safety.
• Determines the care needs and formulates the initial plan of care.
• Communicates with the clinical team to ensure seamless delivery of service.
• Completes enrollment documentation promptly.
• Serves as a community resource for intake issues.
• This is a mobile position with regular travel to different work locations.
• Flexible hours, including possible evenings and/or weekends as needed.
