Job summary

Impact Home Health is seeking an Intake Registered Nurse (RN) in Kansas to conduct initial assessments to determine if services can meet client needs. You will coordinate with care coordinators and internal teams to ensure a timely start of service, while ensuring regulatory compliance and patient readiness.

The role requires 2+ years of RN experience (5+ preferred), an active KS RN license, and an AD nursing degree (BSN preferred). Full-time, no weekend work, on-site opportunity in Wichita.

Qualifications

• Associate's Degree in Nursing required; BSN preferred.
• Active RN license in good standing with the Kansas State Board of Nursing.

Responsibilities

• Collaborate with Intake Coordinator to support timely patient admissions.
• Review referral information for clinical appropriateness and service needs.
• Collect and verify required clinical documentation, including physician orders.
• Communicate with patients, families, and referral sources to facilitate intake.
• Provide clinical input to determine patient care needs and service planning.
• Coordinate with internal staff to support scheduling and initiation of services.
• Ensure intake documentation is accurate and compliant with policies and state regulations.
