Job summary

McLaren Health Care in Davison, MI seeks a full-time Referral Coordinator to ensure timely processing of referrals and intake for services. You will document and coordinate paperwork, enter referral data, and answer inquiries with a focus on accuracy and service excellence.

Responsibilities include coordinating with hospital discharge planners, physicians, clients and family members, and agencies to maintain continuity of care and resolve reimbursement issues.

Qualifications

• High school diploma or GED required.
• Experience with MS Office and related computer programs.
• Associate degree preferred.
• Formal customer service skill training preferred.
• Two years related experience in managed care and home health.

Position Summary:
Ensures that requests for services and procedures are processed in an effective and efficient manner. Completes the intake process which may include coordination of services, data entry of referral information, general inquiries, and other referral and reimbursement issues. Provides a positive experience for the customer and maintain defined service standards.
