Hendricks Behavioral Hospital in Plainfield, IN invites an Intake Assessment Clinician / RN to respond to referrals, assess patients, and support discharge planning. This role requires RN licensure in Indiana and experience in acute psychiatric settings.

As part of the intake team, you will document inquiries, coordinate admissions, and assist on-call physicians, while delivering compassionate care and ensuring a smooth continuum of service for patients and families.

Qualifications

• RN licensure in Indiana required.
• Experience in intake assessment activities within an acute psychiatric setting.
• Knowledge of DSM, Mental Status Exam, and Addiction and Recovery field.

Responsibilities

• Respond to referrals and requests for services and information.
• Document and process inquiry calls according to Intake policies.
• Assess and stabilize patients before discharge or arrange hospital admission.
• Notify and assist the on-call doctor with patient interviews.
• Obtain administrative approval for all admissions.
• Complete admission paperwork following hospital policy.
• Maintain an intake log with all necessary information documented.
• Make follow-up calls to patients who fail to show up for appointments.
• Provide counseling to employees and support training as needed.

You will provide technical assistance to the discharge planning process under the direction of the Director of A&R and offer general support to the case management program.Document and process inquiry calls according to Intake policies.Assess and stabilize patients before discharge or arrange hospital admission.Notify and assist the on-call doctor with patient interviews.Obtain administrative approval for all admissions.Complete admission paperwork following hospital policy.Maintain an intake log with all necessary information documented.Make follow-up calls to patients who fail to show up for appointments.Demonstrate guest relations by example and provide counseling to employees in need of training.Your Experience Matters What we’re looking for:Education & Licensure: A nursing degree from an accredited university is required.
