Job summary

Providence at Home with Compassus is seeking a front-line referrals coordinator to initiate and manage patient referrals, verify eligibility, and input data into the EMR. You will coordinate with referral sources, departments, and insurance teams to ensure accurate and complete information and timely admission scheduling.

The role requires strong problem-solving, multitasking, and communication skills to support home health services and payer coordination in a fast-paced environment.

Qualifications

• Coursework in medical terminology or equivalent experience.
• Bachelor's degree preferred.
• Minimum 2 years in customer service with demonstrated computer proficiency.
• Healthcare setting or home services experience preferred.
• Experience with electronic medical records (EMR) preferred.

Responsibilities

• Coordinates and processes incoming referrals, including assessing needs from other HS departments.
• Evaluates referral information and contacts patient or referral source to determine initial eligibility for HH services.
• Coordinates and ensures insurance information is obtained and recorded into EMR.
• Documents referral information using critical thinking and ensures completeness.
• Triages and coordinates with team members for review if information is incomplete.
• Acts as liaison between referral sources, MD offices, patients/families, and insurance case managers to ensure timely patient care.
