TriHealth is actively recruiting for an RN to join PACE of Cincinnati, located in Norwood, Ohio. The Program of All-Inclusive Care for the Elderly (PACE) is a Medicare and Medicaid program that helps individuals meet their healthcare needs within the community, avoiding nursing homes or other care facilities.
The PACE Intake Coordinator, RN, oversees the intake and enrollment process for new participants. Responsibilities include educating potential participants and caregivers about the program, setting appointments, documenting medical needs, and gathering social and medical information. The role involves conducting home visits, assessing care needs, and coordinating with community and state agencies. The coordinator may also provide tours of the PACE center and must ensure compliance with Medicare/PACE marketing practices.
The role involves frequent sitting, standing, reading, talking, and use of hands, with occasional lifting and reaching. It requires consistent concentration, communication, and reasoning skills, along with visual and auditory acuity.
