Habitat Health is seeking an Intake Coordinator to facilitate the enrollment process for prospective participants in our PACE program. Qualifications

  • Ability to coordinate multiple priorities with attention to detail.

  • Ability to work independently while collaborating with others.
  • Proficiency in Microsoft Office and quick learner of CRM/EMR systems.

  • Deliver a welcoming, participant-centered experience and educate about the PACE model and Habitat Health services.
  • Maintain accurate participant information, referral activity, and intake documentation in CRM/EMR and other systems.
  • Coordinate and track participant requirements through each stage of intake and enrollment to ensure timely completion of documentation and activities.
  • Support high-quality handoffs from intake through enrollment and into the IDT.
  • Assist with community events and enrollment conferences and other initiatives to promote awareness.

Role Scope

The Intake Coordinator plays a key role in facilitating the intake process for prospective participants as they transition into Habitat Health's PACE program. This role coordinates intake activities, including home visits, assessments, and center tours, while partnering with prospective participants, caregivers, and cross-functional teams to ensure a seamless, participant-centered experience. Through proactive coordination, effective communication, and timely follow-through, the Intake Coordinator removes barriers, supports informed enrollment decisions, and contributes to a smooth transition into care.

Core Responsibilities & Expectations for the Role
  • Coordinates the end-to-end intake process for prospective participants by partnering with cross-functional teams to support a seamless and timely enrollment experience.
  • Coordinates home visits, center tours, assessments, and follow-up appointments to ensure timely progression through the intake and enrollment journey.
  • Maintains proactive communication with the Outreach & Enrollment, and clinical pre-enrollment teams to provide timely updates and support efficient coordination throughout the intake process.
  • Partners with clinical teams to identify, coordinate, and facilitate additional assessments required to support participant eligibility and IDT presentation.
  • Delivers a welcoming, participant-centered experience by guiding prospective participants and caregivers through center tours and providing education on the PACE model and Habitat Health's services.
  • Maintains accurate participant information, referral activity, and intake documentation within Customer Relationship Management (CRM), Electronic Medical Record (EMR), and other organizational systems in a timely and accurate manner.
  • Coordinates and tracks participant requirements throughout each stage of the intake and enrollment process to support timely completion of required documentation and activities.
  • Supports high-quality participant handoffs by coordinating a seamless transition from intake through enrollment and into the Interdisciplinary Team (IDT).
  • Supports community events, enrollment conferences, and other organizational initiatives that promote participant engagement and program awareness.
  • Maintains compliance with applicable federal and state regulatory requirements, as well as Habitat Health policies and procedures.
  • Performs other related duties as assigned.
Required Qualifications
  • Bachelor's degree in Healthcare Administration, Business Administration, Marketing, or a related field, or an equivalent combination of education and relevant experience.
  • Minimum of one (1) year of experience in healthcare, enrollment, participant services, care coordination, customer service, or a similar role supporting participant engagement or enrollment processes.
  • Demonstrated ability to coordinate multiple priorities while managing participant information, scheduling, and documentation with strong attention to detail and timely follow-through.
  • Experience working with Medicare, Medicaid, state and county eligibility programs, or other healthcare eligibility and enrollment processes preferred.
  • Strong communication and collaboration skills, with the ability to effectively engage prospective participants, caregivers, and cross-functional teams to support a seamless intake and enrollment experience.
  • Demonstrated ability to work independently while collaborating across teams to achieve shared outcomes.
  • Experience using Microsoft Office and the ability to quickly learn Customer Relationship Management (CRM), Electronic Medical Record (EMR), and other organizational systems.
  • Proof of a valid driver's license, reliable personal transportation, and automobile insurance that meets applicable state requirements, with the ability to travel throughout the service area.
  • Active CPR/BLS certification or the ability to obtain certification within 30 days of hire.
  • Meets all applicable participant-facing health requirements, including TB screening and required immunizations.

Compensation

The anticipated base salary range for this position is $26.00 - $31.00 Hourly . This position is also eligible for a target annual bonus and may be eligible for additional incentive compensation, as applicable.

The final compensation offered will be determined based on factors including, but not limited to, the candidate's qualifications, skills, experience, education, certifications, geographical location, and business needs.

In addition, this position is eligible for a comprehensive benefits package, including medical, dental, and vision insurance; paid parental leave, short- and long-term disability coverage; life insurance; flexible spending accounts; a 401(k) savings plan that includes a match; paid time off; and company paid holidays.
