Job summary

AgeRight Care Management in Milwaukie, OR is seeking a detailed Home Health Intake Coordinator to manage new patient referrals and ensure a smooth start of care. Responsibilities

• Receive and process incoming home health referrals from hospitals and physicians.
• Collect demographics, insurance and medical records for referrals.
• Assist with eligibility, benefits, and payer requirements before admission.
• Enter patient data in the electronic medical record system.
• Coordinate with family, clinicians, and referral sources to obtain missing information.
• Schedule start-of-care visits with clinical staff per policy.
• Track referral status from receipt to acceptance, admission, or decline.
• Ensure compliance with HIPAA, Medicare/Medicaid, and state/federal rules.
• Provide excellent customer service and professional representation of the agency.
• Assist with administrative duties as applicable.

Home Health Intake Coordinator

Summary

We are seeking a detail-oriented and compassionate Home Health Intake Coordinator to manage new patient referrals and help ensure a smooth start of care. This role serves as a key point of contact for referral partners, patients, families, and clinicians. The primary tasks include gathering and entering required documentation, coordinating with the eligibility and scheduling coordinators, and ensuring timely admission to home health services.

Key Responsibilities

• Receive, review, and process incoming home health referrals from hospitals, physicians, and discharge planners.
• Collect patient demographics, insurance information, medical records, physician orders, face-to-face documentation, and other required referral materials.
• Assist with insurance eligibility, benefits, authorization, and payer requirements prior to admission when applicable.
• Enter and maintain accurate patient information in the electronic medical record system.
• Communicate with referral sources, patients, family members, clinicians, and internal team members to obtain missing information and coordinate next steps.
• Schedule timely start-of-care visits with clinical staff based on agency policy and patient needs.
• Track referral status from receipt through acceptance, admission, or decline, ensuring timely follow-up and documentation.
• Support compliance with agency policies, HIPAA, Medicare, Medicaid, private insurance, and applicable state and federal regulations.
• Provide excellent customer service and represent the agency professionally in all interactions.
• Assist with administrative duties as applicable

Qualifications

• High school diploma or equivalent required; associate degree or related healthcare training preferred.
• Previous experience in home health, hospice, medical office, healthcare intake, admissions, scheduling, or care coordination preferred.
• Knowledge of Medicare, Medicaid, private insurance, authorization processes, and home health documentation requirements is a plus.
• Strong communication, organization, problem-solving, and time-management skills.
• Ability to manage multiple referrals, prioritize urgent needs, and maintain accuracy in a fast-paced environment.
• Comfortable using electronic medical record systems, Microsoft Office, email, fax, and phone systems.
• Professional, dependable, and committed to patient-centered service.
• Ability to maintain confidentiality and follow HIPAA requirements.

Preferred Skills

• Experience working with referral portals, EPIC, Wellsky, or other home health scheduling systems.
• Understanding of start-of-care timelines and home health admission requirements.
• Ability to build positive relationships with referral partners and clinical teams.

Schedule and Work Setting

Full-time, onsite position. 8:00 am - 4:30 pm or 8:30 am to 5:00 pm available start times.
