Job summary

Horizon Health in Richmond, IN, seeks an Intake Coordinator (RN/LCSW/LPC) to join Reid Hospital & Health Care Services. You will coordinate referral development and pre-admission assessments in collaboration with the Behavioral Health Program Director to ensure timely access to services.

You will respond to intake calls, complete admission paperwork, conduct assessments, document findings, and communicate with referral sources.

Qualifications

• Bachelor’s degree in chemical dependency, psychology, social work, counseling, nursing or related field preferred
• Master’s degree with practicum in health/human services preferred
• Minimum three years of experience in a similar position and/or industry

Responsibilities

• Responds to intake calls and completes admission paperwork. Assessments completed by RN, LPC or LMSW
• Facilitates groups, family and individual sessions and completes documentation
• Schedules unit assessments, meetings and appointments as appropriate
• Uses assessment tools under supervision of RN/LPC/LMSW
• Communicates with referral sources including family, legal system, psychiatrists, PCPs and other facilities
• Maintains court paperwork and hearings; works with court personnel
• Participates as an active member of the interdisciplinary team
• Ensures confidentiality compliance with state and federal laws
• Identifies improvement areas; participates in performance improvement processes
• Enters data into HorizonPlus and generates monthly referral/admission/discharge reports
• Prepares and submits operational tasks/documentation for corporate offices
• Other duties as assigned

Responsibilities

Intake Coordinator- RN/ Social Work- Behavioral Health Horizon health is seeking an intake coordinator to join our team at Reid Hospital & Health Care Services in Richmond, IN. The Intake assessor, in conjunction with the Behavioral Health Program Director may coordinate the referral development and pre-admission patient assessment process and provide clinical assessments for potential clients.

• Responds to intake calls and completes admission paperwork. Assessments will be completed by RN, LPC or LMSW
• Facilitates groups, family and individual sessions and completing all necessary documentation
• Schedules Unit assessments, meetings and appointments as appropriate
• Utilizes assessment tools including, but not limited to Geriatric Depression Scale, Zung, SLUMS, alcohol screening tool(s) under authorization/supervision of RN, LPC or LMSW
• Communicates with referral sources including, family members, legal system, psychiatrists, primary care physicians, and other treatment facilities.
• Maintains schedule for filing court paperwork and hearings. Works closely with court personnel to maintain good relationships
• Participates as an active member of the interdisciplinary team
• Maintains compliance with state and federal confidentiality laws as well as upholding our Horizon’s confidentiality policies and procedures.
• Identifies areas needing improvement and utilizes the facility performance improvement process. Actively participates in department processes as requested
• Enters required data into HorizonPlus system and generates monthly reports to track and trend referral, admission and discharge data.
• Assists in submission of reports and requests for documents from various regulatory agencies; re: PA Dept. of Health, CAP and Joint Commission
• Prepares and disseminates monthly reports and completes required follow up on activities on behalf of the Director. Prepares and submits all operational tasks/documentation (census report, PIORS) as required by the corporate offices
• Other duties as assigned

Qualifications
