Job summary

Enhabit Home Health & Hospice, a leading national home-based care provider, seeks a coordinator to streamline patient referrals and support smooth transitions to services. You will collaborate with branch and sales teams and apply regulatory knowledge to ensure compliant care delivery.

Eligible candidates bring a high school diploma, 2+ years in health care (1+ year in home health or pediatrics), customer-service experience, Medicare guideline familiarity, and basic mobile tech skills.

Qualifications

• Must possess a high school diploma or equivalent.
• Must have at least two years of experience in the health care industry and one year experience in home health or pediatrics.
• Must have demonstrated experience in customer service.
• Must have demonstrated knowledge of Medicare guidelines that govern a home health or hospice.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

Education and experience, preferred

• A graduate of an approved school of professional nursing and licensed in the state in which they currently practice is preferred; may be a registered nurse or licensed professional/vocational nurse.
• Experience with intake responsibilities and working with a multi-location company is preferred.

Requirements*

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependent transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions* For employees located in Oregon, requirements related to driving are not applicable unless employee has a clinical license.
